UltraCare Specialty Pharmacy takes your privacy seriously. As a pharmacy in Ontario, we are a health information custodian under the Personal Health Information Protection Act, 2004 (PHIPA). This policy explains how we collect, use, share and protect your personal information and personal health information, both in our pharmacy and through ucsp.ca.
Our Privacy Officer is responsible for how we handle personal information and for answering your questions and requests.

When you are our patient, we collect the information we need to care for you safely. This can include your name, address, phone number, email, date of birth, gender, Ontario health card number, insurance and drug plan details, prescriptions, medication history, allergies, health conditions, and notes from your conversations with our pharmacists.
When you use our website, we collect what you choose to send us, such as your name, email, phone number and message when you fill out our contact form. Our hosting and security services also record technical details such as your IP address, browser type and the pages you visit. See our Cookie Policy for more.
We use your information to fill and manage your prescriptions, check for drug interactions and allergies, assess and prescribe for minor ailments, prepare compounded medications, arrange home delivery, bill your insurer or government drug plan, remind you about refills and appointments, and answer your questions.
We also use it to meet our legal and professional duties, such as keeping records required by the Ontario College of Pharmacists and reporting to the Ontario Narcotics Monitoring System when the law requires it. We do not sell your personal information.
When we share your health information with others who are directly involved in your care (your "circle of care"), we rely on your implied consent, as PHIPA allows. For other purposes, such as marketing, we ask for your express consent first. You can withdraw your consent, or ask us not to share certain information with other providers, by contacting our Privacy Officer. Some uses and disclosures are required or allowed by law and don't need your consent. If you limit what we can share, we will explain how it may affect your care.
We share only what is needed. This can include your physician, nurse practitioner and other health care providers in your circle of care; your insurer, benefits plan or the Ontario Ministry of Health to process claims; and our regulators, the courts or law enforcement when the law requires it.
We also use trusted service providers to help us run the pharmacy and this website. These include PharmaChoice, which provides our online prescription refill and transfer tools, along with our pharmacy software, appointment booking, delivery, website hosting and security providers. They may only use your information to provide services to us and must protect it.
Our contact form is not encrypted end to end and is not the right place for detailed health information. Please call or visit us to discuss your health or prescriptions. Never use the contact form in an emergency. Call 911.
We use physical, administrative and technical safeguards that fit how sensitive the information is. These include locked storage and restricted areas in the pharmacy, password-protected systems, access limited to staff who need it, and privacy training and confidentiality agreements for our team. No system is perfectly secure, but we work to protect your information against loss, theft and unauthorized access.
We keep pharmacy records for as long as the Ontario College of Pharmacists and other applicable laws require. When we no longer need information, we destroy it securely. Contact form messages are kept only as long as needed to respond and follow up.
Some of our service providers, including website hosting and security services, may store or process information outside Ontario or outside Canada. When that happens, the information may be subject to the laws of that place. We choose providers that commit to protecting the information they handle for us.
You can ask to see or get a copy of the personal health information we hold about you, and ask us to correct it if it is wrong or incomplete. Please send your request in writing to our Privacy Officer. We will respond within 30 days, as PHIPA requires, or let you know if we need more time. We may charge a reasonable fee for copies. In limited cases the law allows or requires us to refuse access, and if so we will tell you why.
If your personal health information is lost, stolen, or used or disclosed without authority, we will tell you as soon as we reasonably can. We will also report the breach to the Information and Privacy Commissioner of Ontario when the law requires it.
If you have a question or concern about how we handle your information, please contact our Privacy Officer first. We will look into it and respond.
If you are not satisfied with our response, you have the right to complain to the Information and Privacy Commissioner of Ontario:
